At Polly Chapel, we believe in caring for one another. If you are in need of prayer or assistance, please submit your request in the form below. Our team is here to support you with love, compassion, and confidentiality.

Prayer Requests:

Submit your prayer requests so that our prayer team and church family can lift you up.

Care Support Requests:

Request support during illness, bereavement, or special circumstances.

Transportation Requests:

Need a ride to worship service or a church event? Let us know, and we will do our best to assist you.

Transportation Requests must be submitted 2 days prior to pick up date. In the form below, be sure to include in the last section (Transportation Event): The Service or Event Name; Date and Time of event itself (so we can be sure to allow enough time to pick you up and get to the event) and your pickup location.
